We offer PADI instruction for scuba diving lessons and classes for beginners and certified divers, EFR Instructor level classes, EFR CPR/AED/First Aid for divers and non-divers, Divers Alert Network (DAN) classes in the Dallas, Texas and the Dallas Metroplex Area and scuba diving trips to Cozumel and other scuba diving destinations. Instructions:

If you answer "No" to all of the statements on page one, you need to only complete page one. Bring form to class with you.

If you answer "Yes" to any of the statements on page one, complete page one and then take the whole form to your doctor. You will need a doctor's release for scuba diving prior to entering the water. Bring the completed page one and page two of the form including Physician's Statement completed and signed by your doctor to class. The remaining pages are information for your doctor. If you are a doctor, you may not sign the Physician's Statement for yourself.
